We are pleased to announce a new, convenient option for students, staff, and community members to now get the COVID-19 vaccine! USD 456 - Marais des Cygnes Valley will be partnering with Auburn Pharmacy to offer COVID-19 vaccination to anyone 5 years of age or older!

Please use the form below to sign up for the USD 456 COVID-19 Vaccine Clinic. Details about the clinic, including date, time, and location will be communicated once the sign-up period ends.

While the vaccine clinic will be conducted at both MDCV JH/HS and Elementary, vaccination will only be provided to those that sign up, as parents/guardians must provide permission for students to receive vaccinations. This will be a free clinic, there will be no cost to those who receive the vaccine, but a proof of insurance, state-issued identification OR social security number will be required for vaccination. Individuals who sign up for this clinic will receive the Pfizer COVID-19 vaccine, Comirnaty.
