The following is a registration form for in-person worship. This will allow us to know who is attending church and make sure we have enough seating. You are asked to register for each person in your family that will be attending church.  This will give us an accurate count. If you require nursery for children, ages 4 and under, a link will be provided after you submit your form for you to register your children for the nursery.
Each adult attending worship will be required to wear a mask and must wear it throughout the service. Health Acknowledgement Questions will be asked of when you check in for church Sunday Morning.
