Thank you for enrolling in an awesome surf program hosted at Stoked Surf school.

Please answer the questions below and submit to confirm your child's place for surf lessons.

Once your form is sent, and payment is received your Child's place is CONFIRMED

This form is for weekly surfing and or skating with Stoked Surf School and is used to gather information such as emergency contact details, health information and student surfing ability.

One form per child please.

Surfing After School - Mon, Tues, After School 4pm start for 9 weeks, term 4 2023.

Surf Skate Training (Smooth Star/Carver) - Tuesdays 6pm

Saturday and Sunday 9am

Saturday Salty girls 9am

9 Week program. Commences Saturday the 14 of October

Meet 10 min prior to the lesson starting, Cave Rock at the big black trailer opposite 26 Esplanade.

The lesson is for 1 hour and 30 min of organised surf activities. Sometimes Stoked mix up skating, beach flags, and movement session if the surf conditions are variable ie high winds and large seas.
If we have to cancel the lesson due to bad weather ( gale force wind, electrical storm) we will email and text you by 1 pm the day of the lesson. A make-up lesson can be scheduled on another lesson day during the term booked.Lessons do not roll over into the next term.

- Please pay for the session prior to dropping your surfer off.
- Weekly payment can be organised in advance, please send us an email.

Please NOTE Only one make-up lesson is available per child, non-negotiable. If the session is canceled due to bad weather, another day will be offered during the same week or another week during the term.

I acknowledge and willingly assume all risks and hazards in surfing and beach-related activities AND skating from the rendezvous until the conclusion of the lesson.
