GBCS Human Growth and Reproduction Parental Right to OPT-OUT
For HIV/AIDS and sex education instruction, parents and/or legal guardians must be notified in advance of the district program and when instruction will occur.  Parents and or legal guardians have the right to excuse their child without penalty.  (§380.1507, §388.1766)

The Purpose of this form is to OPT-OUT your student from the Human Growth and Reproduction course completely.  It is not possible for students to attend any segment of the course if you OPTION them OUT.  The student will be assigned alternative work in place of Human Growth and Reproduction Unit.
