Adopt-A-Family Sponsor Request Form 
St. Matthew's Adopt a Family Program will be helping to serve 40+ families this year. As a sponsor, you will be assigned a family to help support. Adopt a Family referrals are from two local Renton organizations.

As a sponsor you will:
  1. receive a grocery gift card for the family ( provided by St. Matthews at the time of pick up and delivery)
  2. purchase and provide a gift card(s) to the adopted family so they can purchase their own gifts. 
  3. **deliver to the family (optional)

**As a sponsor, you will have the option to deliver gift and grocery cards to the adopted family, between December 10-17. If you choose not to deliver, an assigned person will deliver for you.
