Please use the following information as a guide: Beginner: You haven’t been on a sailboat before or you’ve been a guest on other people’s boats. Your experience is limited to following directions. Your knowledge of terminology is basic. You have little or no helm time. You’ve been sailing with others, but do not adjust anything without direction. Intermediate: You’ve been sailing with others. You know fundamental terms and equipment and can prepare for and execute maneuvers with brief commands.  You are familiar with basic trim of jib and main, and know when and how to adjust the sails.  Your helm time is limited, but you can read telltales. You know that there are rules of the road, but don't know them well. You have been exposed to navigation but have limited practice.  Advanced: You can hand, reef, and steer and know what each means. You understand navigational terms and recognize all aids to navigation but have limited charting practice. You know how to use a radio.  You can direct a beginner in sail trim. You can use secondary trim controls. You have flown a spinnaker. You can maneuver in close quarters under sail or power. Expert: You can and have sailed in heavy weather both on the helm and handing, reefing and changing sails. You can navigate and chart a course. You can use and program a GPS.  You have skippered a boat.  

I wish to participate in sailing with Lake Michigan Sailing club.  I recognize that sailing can be a hazardous sport that can result in serious injury or death.  I accept the risks inherent in sailing and its environment.  I accept full responsibility for all medical expenses or other claims.  I agree to release, hold harmless and indemnify Lake Michigan Sailing Club, their officers, board of directors, vessels, vessel owners, members, agents and employees for any claims for any injury or damage resulting from any cause, including negligence, which arise out of participation in the activities of Lake Michigan Sailing Club.  This release is binding as to any other persons, including family members, heirs and executors.  I also permit Lake Michigan Sailing Club to use photos and quotes in their publications.

And I, the minor’s parent and /or legal guardian, understand the nature of sailing activities and the minor’s experience and capabilities and believe the minor to be qualified, in good health and in proper physical condition to participate in such activities.  I hereby release, discharge, covenant not to sue and agree to indemnify and save and hold harmless each of the released parties from all liability, claims, demands, losses or damages on the minor’s account caused or alleged to be caused in whole or in part by the negligence of the released parties or otherwise and further agree that if, despite this release, I, the minor, or anyone on the minor’s behalf makes a claim against any of the released parties named above, I will indemnify, save and hold harmless each of the released parties from any litigation expenses, attorney fees, loss liability, damage or cost any may incur as the result of any such claim.
