Confirmation Registration 2019
Please complete the following form to register for the Sacrament of Confirmation.  There is a fee of $150 which covers the costs of the Confirmation Ceremony (certificates, booklets, flowers, Diocesan Fee etc) and also includes the last retreat.  If there is any difficulty paying this fee, please call our office. Link to pay the fee is at the end of the form.  Please also note that completion of all requirements of the Confirmation Prep program is mandatory to receive the Sacrament of Confirmation.
Registration is due by June 1, 2019.

Students must choose a sponsor in accordance with the guidelines.  Sponsor may be a godparent from Baptism, or another person who meets the following: at least 16 years of age, received the sacraments of Baptism, First Eucharist and Confirmation, is not a parent of the child, and is a practicing Catholic (in good standing).
