The Alameda County Transportation Commission (“Alameda CTC”) has issued a Request for Qualifications ("RFQ").

The proposer has submitted, or intends to submit, a response to the RFQ that will list you as a reference. In their request to you, they should have provided instructions and requirements for this process as outlined in this RFQ, including the deadline.

Please complete and submit this Reference Questionnaire by or before the Statement of Qualifications (SOQ) submission deadline. Complete one questionnaire per individual; e.g., if you received requests to provide reference questionnaires for five (5) individuals, you would complete five (5) reference questionnaires, even if such individuals all worked at the same firm, on the same project. Provide as much detail as possible.
