'MMSAC Sports Day - RSVP & Waiver
MMSac EC has organized MMSac Sports Day. Please read the details below and fill this form to confirm yours and your family's participation in this event
Date: Saturday, May 11th, 2024
Time: 8:30 am to 3:00 pm
Venue: Sandra Gallardo Elementary School Playfield, Folsom, CA
MMSac will not provide any safety equipment. Attendees are welcome to bring their own.

ENTRY FEE
  • MMSac Members are FREE
  • $10.00 per Attendee (Non-Members only)
  • $5.00 per Non-resident Dependent of MMSAC member
  • All Kids below 5 yrs are Free
  • Light Lunch will be served
  • Non-members - Please make payment via Zelle to mmsac.treasurer@gmail.com before May 10th, 2024. Alternatively, you can pay using credit card. Click here
Games will be conducted as displayed in the entry Sports Day 2024 Schedule and Entries workbook. 
MMSac EC will set rules and guidelines for each game:
  1. Make sure that you will put down your entries for player and referee in the workbook
  2. **RSVP due date: Tuesday May 9th, 2023**

Waiver, Release of Liability & Assumption of Risk Agreement

  1. In consideration of my participation in this event, I on behalf of myself, my spouse, children and other family members hereby agree to release, waive and discharge MMSac, its EC, volunteers and participants connected to this event of any liabilities.
  2. I acknowledge that this event will include, running, throwing, hitting, catching, and related movements involving known and unanticipated risks which could result in physical injury. Risks will include, but not limited to, being hit by the ball, bat, or another player, falling down, and any other medical injuries resulting from physical activity. I understand such risks cannot be eliminated, despite the use of safety equipment, without jeopardizing the essential qualities of the event. 
  3. I accept and assume all of the risks inherent to this event as described. In addition, if at any time, I believe that I am unable to participate in the event due to physical or medical conditions, I will immediately discontinue participation. I agree to follow any rules and guidelines regulating this event as described by MMSac.
  4. I hereby voluntarily release and discharge MMSac from any and all claims, demands, losses, damages, fees or causes of action (including those arising out of any medical treatment), known or unknown, which are in any way connected with my participation in this event.
  5. I acknowledge that I have adequate insurance to cover any injury I may suffer while participating in this event, or else I agree to bear the costs of such injury or damage myself. 
  6. I acknowledge that I have no medical or physical condition which could interfere with my safety in this event, or else I am willing to assume and bear the costs of any and all risks that may be created, directly or indirectly, by any such condition. 
  7. I agree that Marathi Mandal Sacramento is not liable for picture(s)/video(s) being taken during the Holi event and shared online or on any social media platform(s).
