This form serves as registration for the WesternU, PNWU, ICOM Multi-COM Information Session being held on May 8, 2020 from 3-5pm(PST) via a Zoom video conference call.  Only your name and email are required, but filling out the full form is appreciated.  If you are a prospective student the additional information will better allow the three institutions to stay in touch with you, and let us keep track of your contact with us prior to applying.  If you are not a prospective student, you are only expected to provide your name and email address.  You may select that you are a pre-health advisor or other in the university box if wished.  Everyone who registers will be sent an email with a link to the Zoom conference call after filling out this form.
