DCHC School/Sports Physicals Summer 2023 Events 
Douglas County Health Center (DCHC) will be hosting school/sports physical events at multiple Douglas County communities Summer 2023. By hosting these events, we hope to reduce barriers of access to Douglas County students and their families for required school/sports physicals for the 2023-2024 school year. 

DCHC staff is scheduled to be in the following towns on the following dates and times:

Friday, June 2, 2023: Arthur Community Room 10:00am - 5:00pm
Friday, June 9, 2023: Newman City Hall 10:00am - 5:00pm
Friday, June 16, 2023: Arcola Center 10:00am - 5:00pm
Friday, June 23, 2023: Villa Grove High School Gymnasium 10:00am - 5:00pm
Friday, June 30, 2023: Tuscola Community Building 10:00am - 5:00pm

DCHC will be utilizing a flat fee system for students who may choose to avoid private insurance billing or may be uninsured for the event. The fees associated at most will be school physicals ($10) and sports physicals ($5) per student. Funding is available to cover the cost of services for anyone with inability to pay. If a student participates in an Illinois Medicaid plan or would like DCHC to bill their private insurance for the school/sports physical, DCHC staff may ask for the date the student last received a school/sports physical due to annual insurance billing limits.  

Upon completion of this form, DCHC staff will contact parent/guardian at the phone number provided to schedule a time on the above dates for parent/guardian or someone they designate to bring the student to the appointment for their school/sports physical. 

By continuing to complete this form, you acknowledge you are the parent/guardian of a student or a student (aged 18 years or older) from one of the following school districts: Arcola CUSD #306, Arthur CUSD #305, Shiloh CUSD #1, Tuscola CUSD #301, or Villa Grove CUSD #302 and would be interested in registering for a community school/sports physical event in Douglas County in Summer 2023.
