This waiver and declaration (the “Declaration”) must be completed prior to visiting properties managed by Crown Property Management Inc. (“CPMI”). A verbal declaration will also be required on every visit to CPMI buildings.

CPMI appreciates your cooperation in following the COVID-19 protocols that have been established to protect all Tenants, Staff, Contractors, and our Community. These protocols have been established based on recommendations from the Province of Ontario and health and safety association guidance.

Access is hereby provided based on the assurances made herein by all service providers, contractors and building operators, made to CPMI and to each other, on each visit to CPMI buildings. Further, service providers, contractors and building operations agree to observe and fully abide by all rules, protocols and procedures set out by CPMI. As we continue to monitor the situation regarding COVID-19 in Ontario and Toronto, we will continue to review protocols and procedures and adjust accordingly.

The virus that causes COVID-19 can be transmitted by asymptomatic people and there is a risk of transmission despite all of CPMI’s best efforts to impose and enforce appropriate safety protocols. Your information and honesty are important for the health and safety of everyone you come into contact with.
