Concordia Environmental Stewardship Survey
INTRODUCTION
You are invited to complete a questionnaire to determine attitudes toward environmental stewardship at Concordia University Texas. Please take whatever time you need to discuss the questionnaire with your family, friends, and peers. The decision to participate is entirely up to you, and you may end your participation at any time.

In this study, we are evaluating the following things:  1) knowledge of and attitudes toward key environmental issues; 2) awareness of and attitudes toward existing campus environmental practices, programs, and amenities (defined as desired or useful buildings or structures); and 3) the impact of Concordia’s natural setting and environmental practices, programs, and amenities on the well-being of students, faculty, and staff. The results of the study will be used to guide future environmental stewardship activities on campus in an effort to improve quality-of-life for the Concordia community.

WHAT IS INVOLVED IN THE STUDY
If you decide to participate you will be asked to complete the questionnaire in its entirety. We think this will take you about 10 minutes. If you feel uncomfortable completing a particular part of the questionnaire, you may choose to skip it. You may choose to end your participation completely at any time. Your responses will be added to other responses, analyzed, and used to inform campus planning and programming. Your name and contact information will be used to enter you into a drawing for one of several gift cards, but you may choose not to enter the drawing by withholding your name and contact information.

RISKS
There are no identifiable risks involved in participating in this study. Your personal information will be kept confidential and will be used only to contact you in the event you win one of the prize drawings.

BENEFITS TO TAKING PART IN THE STUDY
By participating in the study, you can help guide decisions on how we use and develop the natural areas of our campus. Our goal is to increase outdoor leisure and recreation opportunities for the campus community and general public, and your responses will help us determine the most effective means to do so.

CONFIDENTIALITY
Your information will remain private and confidential. Only staff from the Office of Environmental Stewardship and faculty from the Department of Environmental Science and Conservation will read your responses. Your contact information, should you choose to provide it, will be used only to inform you in the event you win the drawing for the gift cards. Upon completion of this project, your responses, including contact information, will be destroyed.

INCENTIVES
Participants may choose to enter into a drawing for one of several prizes. You may choose not to enter the drawing by withholding your email address. If you win, you will be notified via your Concordia email address.

YOUR RIGHTS AS A RESEARCH PARTICIPANT
Participation in this study is voluntary. You have the right not to participate at all or to leave the study at any time. Deciding not to participate or choosing to leave the study will not result in any penalty or loss of benefits to which you are entitled, and it will not harm your relationship with Concordia University Texas.

TIMELINE
The questionnaire is open until Friday, April 14.
