By now you have reviewed our class information in our website (www.danceataim.com). In this form below, you will input your family information and what classes you'd like for your child to join. When you complete this form, you will be sent via email our waiver and release form which will give you detailed information on our policies. Please review, sign and return the waiver by your child's first class. Your child will not be able to participate without this form signed by you.

To register your child into our dance classes, answer the questions below. If you have more than one child registering, please complete an additional form (one form per child).
