Before submitting your form, please remember:

1. If you are not registered for the convention, your submission will not be accepted.
2. You must volunteer for two shifts (six hours total) in order to qualify for reimbursement of the convention registration fee.
3. All volunteer shifts are first come first served.
4. CSHA staff will keep a volunteer waiting list. If a slot opens, and you are next in line you will be contacted.
