FAYETTEVILLE - Enrollment Change Request Form
To make a change or to cancel enrollment in the school, the Account Manager must complete this form and submit. Once submitted, it will be time-stamped.  We will promptly process your change or cancellation request. For cancellation of enrollment, you will be billed for 30 days from the date you submit this form or until attendance is scheduled to end (whichever is greater).  Please note, a separate form must be processed for each student change or cancellation.

Before cancelling enrollment, please consider that taking breaks in music development leads to a regression of skills and muscle memory that must be relearned and redeveloped.

Students who re-enroll after cancelling enrollment will incur a $39 reactivation fee. Students returning after leaving for a calendar year or more will incur a re-enrollment fee of $99.
