All participants must complete this form to accept that they have read the guidelines for a safe return to lacrosse and understand the risks associated.
This is NOT a waiver where you sign all your rights away to us for life and we drain your bank account, it is just a statement that says you know the Covid rules and that playing lacrosse comes with an elevated risk of contagion.
