Pupil Home LFD test - Results Log
Please complete the following form to report the LFD test result for your child.

You are asked to complete two LFD tests at home per week. We ask that you complete these tests each week on a Sunday and a Wednesday. Please ensure that you contact the school if your child tests positive as well as completing this form.
(If for any reason you miss a test on a Sunday, complete it on Monday and do the next test on the Thursday of that week. Revert back to Sun/Wed for the following week)
