Are you interested in performing a solo, duet or small group dance outside of your regularly scheduled classes? (Please note: Additional performance opportunities will require private lessons to learn and master choreography)

Please confirm how you would like the remaining balance due for costumes to be completed. Your exact costume fees will be communicated to you with five (5) business days of submitting this form and are due no later than March 31st, 2022. *Late registration costume fees will be charged immediately to the account on file by default.

I understand that payment amounts are specific and unique to each individual performer as others may be participating in additional routines and thus have a different expense than another dancer. I understand that the total balance due will be confirmed after sign-up and that all monies must be paid in order to receive costumes, purchase tickets, and perform in the show. I hereby give permission for my child(ren) to participate in Steppin' Out Ballroom Dance's 2022 Graduation Ball. I have read and understand all requirements, policies, payment amounts, and due dates. I understand that there are no refunds and that if I decide to withdraw I am still obligated to pay the full balance due. I understand that if payments are not paid by the listed due dates that costumes and participation may be withheld. I understand that there are no refunds should the show be rescheduled or reimagined due to the ongoing COVID-19 pandemic.
