Please read this form and indicate if you consent to your student receiving Mental Health programming and services during the 23/24 school year at MHS.
The MHS Mental health team includes:
School Social Workers (SW)
School Psychologists (SP)
Prevention/Therapeutic Specialists (PTS)
School Counselors (SC)
We are requesting your permission for one or more of the following:
The BESS - The Behavioral & Emotional Screening System: ALL DPS students participate in this Screener 3 times per year (Sept, Jan, April). This screener helps us identify students who may need additional social/emotional support in order to be more successful in school and life.
Suicide Prevention programming: Mandatory for all 9th and 12th graders every year. Ninth graders will receive "Signs of Suicide" at Ninth Grade Academy in August and 12th graders will receive "It's Real" during Spring Semester.
Individual counseling with the School Social Workers or School Psychologists, if needed.
Group counseling services with SW, SP, PTS, or SC, if needed.
Assessment of your student’s performance/functioning (including observations, consultation, and progress monitoring)
Substance Use Prevention/Intervention Programming (individual, group, and/or class wide)
For Individual and/or Group counseling consent:
We are requesting your permission to provide group/individual counseling services for your child, during regular school hours at your child’s school, if needed. These services are being offered at no cost to your child. The purpose of the service is to support the social, emotional, and/or academic well-being of your child.
Groups are available around the following topics: Coping Skills, Anger Management, Mindfulness, Conflict Resolution, Substance abuse, Marijuana Education, Grief, Depression, Anxiety, Emotional Regulation, Decision Making and Trauma.
Included with groups will be an assessment in the beginning and end of the group to determine the level of need and amount of growth. This can include: BESS, NOMS, BYI-2, BASC-3, and SBIRT.
Every effort, within the law, will be made to keep your child’s information confidential, but information may be shared with other school professionals, as needed, in the best interest of your student’s academic and social/emotional needs.
If your student needs a higher level of social/emotional intervention or support, the provider will reach out to you to discuss options and more specific consent at that time.
Note:
ALL school employees are Mandatory Reporters and are required to report suspicion of danger to self or others, child/elder abuse or neglect, harm or exploitation, and/or possession of controlled substances or weapons on school grounds.
