Thank you for your interest in St. Paul the Apostle, Kingston's Children's Choir.

The Children's Choir will sing the last Sunday of every month at the 9 am mass.

There will be one practice on the 3rd Monday of every month from 6-7 pm at the Church.

Children ages 7+ are welcome to join us!

Please complete this permission form for participation in this year October 2023 - June 2024.

Parents are encouraged to stay for the practice but there will be volunteer supervisors that have been screened by the parish.

I/we understand that reasonable precaution will be taken to safeguard the health and safety of the participant and that the designated emergency contact person will be notified as soon as possible in case of emergency. In the event of any sickness or accident person(s) will not hold St. Paul the Apostle Parish, the Archdiocese of Kingston, or any parish volunteer responsible.

I/we authorize and consent that emergency treatment be rendered under the general or specific supervision and on the advise of any physician or surgeon, licensed to practice in the Province of Ontario. The undersigned understands every effort will be made to notify the parent/emergency contact in the event that treatment is necessary.

In signing this registration, I am granting my child permission to participate in St. Paul the Apostle Parish Children's Choir at 1111 Taylor Kidd Blvd, Kingston

I understand that my child's photograph and/or likeness and name may be used in parish publication, website and livestream video publication. 9 am masses at St. Paul's are livestreamed on Vimeo and Facebook.

Please ensure that you come inside to pick up your child should you drop them off for practice. Parents are encouraged to stay for rehearsals if able.

I/we acknowledge that my child will be respectful and participate in choir rehearsals and masses. Should behaviour become an issue, appropriate actions will be taken in consultation with the Choir Coordinator, Parish Priest and parents/guardians to ensure that child and peers are both safe and enjoying their experience with the Children's Choir.
