SCIS Sky Ranch Camp Student Sign Out - Alternate Transportation Release Form for a ride home on February 10th

All students are required to ride the school bus to Sky Ranch on February 8th. We would like to know how your child will get home from Sky Ranch on February 10th. If your child will ride the school bus back to SCIS there is no need to complete this form. If you would like them to ride with an approved chaperone, we must have a completed form by January 9th in order for your child to not ride the bus back to SCIS. We will only order enough buses for students who are not riding with a chaperone, so it is important for us to have an accurate count. Thank you!

I plan to have my child ride home with the above chaperone from Sky Ranch on Friday, February 10th instead of them riding the school bus. Therefore, in consideration of this transportation arrangement, I waive any claim of liability or responsibility and covenant not to sue Lovejoy Independent School District, its board members, officers, directors, agents, representatives, and employees in the event of an accident causing injury to the above named student which may result from such transportation. I further release, acquit and forever discharge Lovejoy Independent School District, its board members, officers, directors, agents, representatives, and employees from any and all claims that may be asserted by my child arising out of any accident which may result from such transportation. I further agree on behalf of myself and my child to defend, indemnify and hold harmless the Lovejoy Independent School District, its board members, officers, directors, agents, representatives, and employees from any and all losses, costs, or expenses, incurred as a result of any accident or injury that may occur to my child as a result from such transportation.
