High School Boys Tryout Form
This form is only to be filled out by new players coming to our tryouts.  Existing club players are not required!

New players are required to register to attend tryouts. While last-minute walk-ins will be available on-site, we ask new players to register by Wednesday, July 28th, to ensure proper planning.  Walk-ins require to show up 30 minutes early to check-in tent to complete the registration process.

At field arrival, every player needs to go to the check-in tent to get a tryout number that will be used to identify and evaluate the player throughout the tryout process.

All players will be assigned to their age group team coach and a tryout field number.

All players must come fully equipped: Soccer gear, shin guards, soccer cleats, plenty of water, and a soccer ball.
In case of inclement weather, session make-ups will be Friday, June 4th, at the same scheduled times.

Parents are encouraged to stay off the fields. They are only allowed to watch from the fence or parking lots.

Tryouts are FREE!

IMPORTANT: Team placement offers will be sent via email right after the second tryout session. The acceptance window will be limited to 24-48 hours from receiving the email offer.  Players will need to register and pay in full to secure a spot on the team.

RELEASE AND WAIVER OF LIABILITY & RESPONSIBILITIES
I understand that playing or participating on any CISA camps, training session, tryouts or any other club activities may be a potentially dangerous activity involving risk of injury.

I understand that in any contact sport, such as the soccer involved at this tryout event, an athletic participant can be seriously injured. I am aware that the dangers and risks of my child's playing or participating in the above sport include, but are not limited to, falls, contact or collisions with other participants, equipment and facilities, and the effects of weather, including high heat and humidity (facilities are not air conditioned).

I have certified to the club director that my child is in good health and physical condition and sufficiently able to participate in the above sport and the tryouts.

I have advised the director of any limitations on my child's activities for medical reasons in writing below. Knowing and having been informed of the potential dangers and risks associated with playing the above sport, and in consideration of my child being allowed to participate in the camp,

 I fully understand that the participant will be held responsible for all property damage.  This Release and Waiver extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown.

I hereby consent to permit the coach and staff working at the Colorado International Soccer Academy  to provide emergency first-aid or medical treatment for my child, according to their best judgment, in the event he/she suffers an injury or illness while participating in the tryout event or on the school premises.
 
CISA is not responsible for any personal items that are lost, stolen or damaged. I also understand that any pictures taken at this event may be used in any promotional materials.
