This form is needed for all user groups to be filled out completely. Please note, users that are not running a NIS program require 5m liability. Upon Completion of this form and requirements you will receive access to our shared Schedule. Block (large user groups, or weekly) scheduling may be done by contacting us directly, now, or upon completion. *PGMHA team rentals are not required to show proof of insurance.

The Restaurant is not operated by NIS--It's great-- and it serves liquor, but please CONTACT PGGCC for hours and availability to serve your groups needs:

Users require 5m in liability, and proof of liability insurance. (NIS events are covered by our insurance, user fees may occur and PGMHA events/rentals are already insured). If you don't have insurance consider below, OR CONTACT US!!!

Integris Insurance has helped us out!!! Try them for any of your Insurance needs. WOW!
