Booking Terms and Conditions

This booking form is to attend the online ASK workshop: Assessing for Suicide in Children.

The ASK workshop takes place over 9 hours, across two consecutive days from 9am - 1.30pm (GMT) using zoom. Participants must attend for the entire duration of the workshop.

Participants are asked to read pre-workshop material and complete an assessment, sent a week prior to the course date.

Participants will be required to complete an evaluation form immediately following the workshop.

A confirmation of your booking will be sent to you following your completion of this booking form.

The cost of attending ASK online is £85.50 per person paying by card, £83.50 when paying by bank transfer.

Payment must be made within 30 days of booking your place or by the course date, whichever is closest.

Booking Agreement

I understand that Mental Health Learning could cancel this session at short notice if necessary. I understand that Mental Health Learning is not liable for any financial costs incurred by a participant for these cancellations / alterations. If a cancellation of a workshop is decided by Mental Health Learning and an alternative date cannot be agreed between MHL and delegate, a full refund will be given to the delegate.

I understand that it is my responsibility to contact Mental Health Learning if I do not receive an acknowledgement / confirmation of this booking.

I agree to my details being added to Mental Health Learning's database so information relating to my booking on this session can be sent to the contact details I have specified above. (If you have selected no to our keeping of your details above, your record will be deleted after the 6months evaluation form has been received).
