2021-22 Club Wildcat After-School Enrollment Request Form
Scottsboro City Schools is committed to serving students and parents no matter if it is during school or after school, while at the same time, maintaining as safe an environment as possible for our community as a whole. Club Wildcat after-school program will follow the SCS 2021-22 Academic School calendar.

The 2021-22 Club Wildcat after school activities will follow the guidance from the Alabama Department of Health (ADPH), the Center of Disease Control (CDC), and other federal and state recommendations concerning best practices. While we cannot, nor can anyone, anticipate every challenge that may lie ahead for us in 2021-22 school year it is or goal to carry out clear, actionable steps that can be implemented by our faculty, staff, administrators and students. It is our goal to return to as normal as possible for the 21-22 school year.

CLUB WILDCAT AFTER SCHOOL ENROLLMENT INSTRUCTIONS BELOW:

IMPORTANT....Completing this form does not guarantee a spot into the CLUB WILDCAT after-school program.

*2021-22 Club Wildcat after school program is for students who have chosen the Traditional K-6 option for the 2021-22 school year.  

*Only the Parent/Guardian of the student is allowed to complete the Request for Enrollment Form.

*Completing this form ONLY guarantees that your child will be put on a CLUB WILDCAT enrollment request list. Completing this enrollment doesn't secure you a spot into the Club Wildcat after-school program. However, it is our goal to have enrollment into to the 2021-22 Club Wildcat program completed no later than June 1, 2021.
 
*Carrie Bradford will contact parent and/or guardian to secure your child's spot in CW After school program.  Once you have received acceptance into the program a formal application will need to be completed and a paid registration fee of $25.00/per child will secure your spot for the 2021-22 school year.

*If you do not receive a phone call your child or children will automatically be listed on a waiting list for availability throughout the 2021-22 school year as it becomes available. Carrie Bradford will contact you as positions become available.
