Please submit this form to cancel or change enrollment for Madison Avenue Dance Studio Class(es).  Please submit cancellation requests by the 15th of the month in order to drop for the following month. If submitting cancellation form after the 15th of the month, you will be charged tuition for the following month.  *NOTE: student is welcome to attend classes for the duration of the paid period. I further understand that I will be charged a $50 Early Cancellation Fee if I drop out of classes anytime after November 15 (this fee will be automatically charged to your card on file on the date your class cancellation is processed).  All tuition and costume fees that have already been paid at the time of my cancellation are non-refundable. *
