PSSA Opt-Out Information
Dear Parents and Guardians,

If you are interested in opting your child out of state testing due to religious reasons, you are required to come to Fugett Middle School to review the exams and to confirm your opt-out status. Below, you will be asked to verify that you have read the PDE confidentiality agreement. You are also required to select a date and time from the dates listed at the bottom of this form to come to Fugett Middle School to review the exam with Dr. Mitchell. 

Appointments for opt-out reviews are available April 14th, 17th, or 19th. The opt-out process takes approximately 15 minutes to complete.

You must complete one of these forms per child if you have multiple children.  

Please arrive promptly to review and complete the opt-out process with Dr. Mitchell!

Please note:

1. You are required to place your cell phone in a designated area inside the review room until your review of the exam is completed. 

2. Once you complete your review, you will be asked to scan or take a picture of the request for opt-out form and send it via email to Dr. Reynolds, Substitute Superintendent of Schools, for her review and approval. Dr. Reynolds is the only person that can approve your request to opt-out of PSSAs. Specific instructions will be given during the review session.

Appointment Selection
Please select an appointment date and time using the calendar below. Appointments are provided on Friday, April 14th, Monday, April 17th, and Wednesday, April 19th from 8 am to 2 pm. Appointments will take 15 minutes to complete.
