Thank you for your interest in registering for Confirmation preparation at St. Mary Parish in Gilroy. The following form is meant to be completed by both the Candidate (student) and parent. It should take about 10 minutes to complete this form. All information in the form is required in order to be fully registered in the program.

This Confirmation program is for all baptized 9th, 10th, and 11th grade students. If the students is in 8th grade or 12th grade, please DO NOT fill out this form. Instead, please call the parish office to learn about the different options available for sacramental formation.

Before you begin, you should have the name and address of the church the Candidate was baptized in, as well as the date of the baptism (you can find this in the Candidate's baptismal certificate).

This online form, along with the 'Student Activity Waiver,' and a $75 deposit are due in the parish office no later than August 1, 2019 (The total registration fee for Confirmation Year 1 is $150, which is due April 17, 2020). Please read the instructions and questions carefully and complete the entirety of the form. Please submit one form per child.

The 'Student Activity Waiver' can be picked up at the parish office or at the parish website ( www.smpgilroy.org/conf ). This waiver should be dropped off at the parish office or emailed to carlos.barba@dsj.org

*There will be a mandatory Parent/Candidate Orientation on Tuesday, August 27 at 7:00pm in the Church.

Gracias por registrarse para la preparación para la Confirmación en la Parroquia Santa Maria (Gilroy). El siguiente formulario debe ser completado tanto por el Candidato (estudiante) como por el padre. Debería tomar unos 10 minutos completar este formulario. Toda la información en el formulario es requerida para estar completamente registrado en el programa.

Este programa de Confirmación es para todos los jóvenes que han sido bautizados, y están en los grados 9, 10 y 11 de la preparatorio (high school). Cualquier estudiante que este en el grado 8 o 12 debe llamar a la oficina parroquial para informarse sobre sus opciones de formación sacramental.

Antes de comenzar, debe tener el nombre y el domicilio de la iglesia en la que se bautizó el joven, así como la fecha del bautismo (puede encontrar esto en el certificado de bautismo del Candidato).

Este formulario, junto con el 'Student Activity Waiver Form' y un depósito de $75 se deben entregar en la oficina de la parroquia a más tardar el 9 de agosto de 2019 (el total de inscripción para el Primer Año de Confirmación es de $150, que se debe entregar antes del 17 de abril de 2020).
