Spring Trip Survey
Please complete this form if you intend to travel to Silver Dollar City with the Band on Saturday, May 7th. This trip is optional for everyone except Jazz Band Students as the Jazz Band will be performing in a festival prior to going to the park.

If you answer YES that you are attending and that you do NOT have a season pass, we will be ordering a ticket for you and you will be responsible for paying for it.

I have a 2022 Silver Dollar City Season Pass *
If your student has a Season Pass they will only need to pay $10 for the transportation.
I plan to ride the bus back to WJHS after the trip. *
Families that travel with us may check their student out from the park and the student may ride home with their family. Please let us know so we can plan accordingly.
I have a cellphone that does NOT require wifi to operate *
Students will sign up to be in groups of no fewer than 3. Each group MUST have at LEAST 1 student with a non wifi dependent cellphone for communication purposes throughout the day.
If you need additional tickets for family members, how many do you need?
Cost of each additional ticket is $72 each if ordered with us.  Cost to purchase a ticket at SDC is $83 each.
How much do you owe for your ticket purchases? $72 X number of tickets from previous question.
