We are excited that the Grade 6/7 Badminton Team will be participating in the CESS Badminton Tournament on Wednesday, April 20th at Shuttlesport Badminton Academy in Port Coquitlam, BC.
Hope Lutheran Christian School requires each student participating in an athletic event to complete and return the written consent from their parent(s)/guardian(s) in order to participate. Unlike other tournaments hosted at schools, Shuttlesport Badminton Academy has a separate waiver (
https://www.shuttlesport.com/waiver) that you will also be required to complete prior to your child competing in this tournament. In addition, the school seeks to advise parent(s)/guardian(s) of potential risks of injury associated with the athletic event. Due to spacing

Date of Event/Meet: Wednesday, April 20th, 2022
Students will need to bring: HopeLCS PE Shorts, HopeLCS PE Shirt,
Court/Indoor Shoes (NON-MARKING), Badminton Racquet(s), Lunch/Snacks, Water
and Masks. They will be required to be worn UNLESS you are actively playing a game!
Arrival Time:
Wednesday, April 20th: 8:30am at Shuttlesport Badminton Academy (Parents MUST
drop off students at Shuttlesport Badminton Academy!
Expected return times:
Wednesday, April 20th: 12:00pm at MS Campus (3180 Flint Street, Port Coquitlam)

By checking the "Yes" box below I give permission for my child to participate in the athletic event as described.
I acknowledge that, while the School Staff and Badminton Coach will take reasonable steps to prevent injuries to students, some degree of risk is inherent in the nature of this activity, and may occur without fault on the part of the student, School Board, its employees or agents, or the facility where the activity is taking place. By allowing my child to participate in this activity, I am agreeing that the activity described above is suitable for my child, and I am accepting the risk of injury associated with the activity.
My child and I understand that the Student Code of Conduct applies during the field trip, and I will be responsible for all costs incurred due to my child’s failure to abide by the Code of Conduct (i.e. cost to send my child home).
