Epiphany CYO Basketball Registration (2024-2025)

Registration is now available for the 2024-2025 Epiphany School CYO Basketball Program.

Eligible Players: Epiphany students and registered parishioners in grades 5-8.

CYO Regular Season: Dec. 2, 2024 - Feb. 9, 2025.

CYO Playoffs and Championships: Feb. 10-12, Feb. 22-23, and March 1-2.

We aim to have boys' and girls' teams for each grade. Open gym workouts for each team are currently ongoing and the schedule is below. If your child is interested but still unsure whether they wish to participate, we encourage them to attend an open gym and meet the coaches and current players.

The deadline to register is October 11th. After registration ends, your coaches will communicate future practice/game schedules and attendance expectations. Once the season begins, practice and game times will vary and occur during the week and on weekends.

The registration fees are as follows:

$375 per player if you need a uniform (this includes all 5th graders and new players in our program)
$325 per player for returning players in grades 6-8 who plan to use their uniform from last year.

Checks should be made payable to The Epiphany School and sent to the attention of Mrs. Rose Gallagher. For uniform size determinations, please consult the uniform size chart, which is also attached to this email.

The number and size of teams is dependent upon the number of registered participants. As an accurate count of players is crucial to assembling teams and scheduling gymnasium times, any late entries will be placed on a “waiting list,” and will only be afforded the opportunity to participate should a registered player drop out of the program.

Consent for participation in Epiphany CYO Basketball *

I hereby give consent for my child, named above, to participate in the Epiphany CYO Basketball Program for the 2024-2025 season. I understand that participation in this athletic program involves certain risks and hazards, including but not limited to, physical injury or harm. I accept these risks on behalf of my child.

Player's physical well being and care *

I hereby certify that my child is physically able to participate in basketball. I authorize the school, league, and its representatives to obtain medical treatment for my child in case of emergency. I understand that a refund will not be issued should a future injury or illness prevent my child from completing her/his participation in the season.

Liability Waiver *

I understand that The Epiphany School, Epiphany Parish, and CYO program volunteers will take reasonable precautions against accident, personal injury, and loss of, or damage to, property, but they, the coaches, the Catholic School Region and/or the Archdiocese of New York, as the case may be, are not assuming any legal liability for any such occurrence except any liability based on their failure to take such reasonable precautions.

Please submit all registration fees to Mrs. Rose Gallagher by October 11, 2024. Checks should be made payable to "The Epiphany School."

Envelopes should be marked "CYO Basketball" with your child's name & grade in place of a return address.

Registration will not be considered complete until payment has been received.
