I hereby release and waive any liability against Chinese Christian Herald Crusades (known as CCHC), Herald Family Center (known as HFC) and its officers, employees and volunteers for any illness or injury that my child may incur as a result of his/her participation in the enrolled program. Should it be necessary for my child to have medical treatment, while participating in the enrolled program and its activities, I hereby give CCHC/HFC and/or its agents permission to act on my behalf to secure any hospitalization or medical services deemed necessary and appropriate by the physician and absolve CCHC/HFC from any and all forms of negligence and wrong treatment incurred in the procurement and process of hospitalization and medical treatment. CCHC/HFC has my permission to obtain emergency medical treatment for my child at the expense of myself and/or my insurance company. 在此我同意，如果我的子女因為參加所登記的計劃活動期間生病或受傷，我放棄追究基督教角聲佈道團 - 角聲家庭中心及員工和義工的責任權利。如果本人的子女在參加所登記的計劃活動期間需要接受治療，本人授權基督教角聲佈道團 - 角聲家庭中心或其代表代替本人提供醫生認為有必要的住院或醫療服務。基督教角聲佈道團 - 角聲家庭中心將不負責在醫院治療當中疏忽或誤診之責任。本人授權基督教角聲佈道團 - 角聲家庭中心為本人的子女提供醫療急救，費用由本人或本人的保險公司承擔。
