We are working with Seattle Public Schools Transportation about the possibility for students to take the bus from their Elementary School on Wednesdays and be dropped off at Aki Kurose for Cadenza. Depending on the availability of busses, this may or may not be possible for your student's school. We are doing our best to make this option available. More information will be shared as this develops. Please select all of the options below that best apply to you situation: *

Media Release Statement
Seattle Youth Symphony Orchestra and Cadenza After School Orchestra Program strives to be a fixture of the vibrant Southeast Seattle community. Sharing with the community and documenting our activities through photography and videography will inform the public and our donors of the amazing work we do and help uplift the ensemble and its musicians. Photos and videos will only be used by Seattle Youth Symphony Orchestra to promote and celebrate Cadenza and its members, with the upmost respect for everyone involved. If you have any questions about this, please contact daniel.mullikin@syso.org. Please read and respond to the following statement about media release:

I give my permission for my student to be photographed, videotaped, and/or interviewed by representatives from Seattle Youth Symphony Orchestra.

I turn over all ownership of the footage and recording to Seattle Youth Symphony Orchestras, and waive any right to inspect or approve the finished videotape or interview, the soundtrack, script, or printed matter that may be used in conjunction with them.

I understand and consent to have this footage used in a public manner, which could include, but it not limited to, among other uses, posting on public websites and social media, viewing at conferences and other meetings, appearing in promotional e-mails, and excerpted in print publications.
