Please fill out this form to submit a student essay to the 2021 Illinois Fatherhood Initiative Fatherhood Essay Contest. This form will gather student information and allow the upload of the official 2021 IFI Fatherhood Essay Contest entry form. The deadline to submit this form is midnight on March 19, 2021.

After the student has copied and edited this file, and the father-figure has added a response, this form can be used to enter the file into the contest. This form is intended to be filled out by either the Student's teacher or other legal guardian and not the student themself. An email account is required to submit this form.

1. Essayists will receive a Certificate of Participation
2. 130 semi-finalists (10 per grade - including Preschool / Kindergarden) will be invited to attend a special recognition event with their honoree.
3. 12 essay finalists will receive additional recognition and be invited with their honoree to attend the 2021 IFI Fatherhood Dinner Celebration in June 2021.
