The use of this special form for documenting unusual bird sightings is not necessary, but you might find it useful. You may fill out and submit the online documentation form below, or download a form from the OOS website. Photographs and/or sketches are particularly useful in documenting bird sightings in addition to written descriptions. For tips on documenting your sighting, click here.

Tip: For those entries with a lengthy response, it is recommended you compose those first in a word processing program, and then copy and paste them into the form, so you do not loose your work if you inadvertently navigate away from the page.
