PARENTAL COMMITMENT I will faithfully bring my son to class promptly at 3pm, which I understand is vital for his success in the program. I understand the financial commitment is as follows: Registration $125 non refundable. 1st to 5th total of $600 and 6th to 11th is $800. NOTE: Please do not submit any payment until you receive an acceptance letter.

OPTIONS due by August 5th
1 Payment = $600 due by August 5th
2 Payments of $300 due July 1st, August 5th
3 Payments of $200 due June 3, July 1st, August 5th
Or you can make payments at anytime as long as the amount in FULL is paid by August 5th.
SIXTH to ELEVENTH GRADE:
OPTIONS due by August 5th
1 Payment = $800 due by August 5th
2 Payments of $400 due July 1st, August 5th
3 Payments of $266.67 due June 3, July 1, August 5th
