Please fill the following form in if you fit into any of the key workers categories which are parents who are critical to the Covid-19 response with children who cannot safely be cared for at home.

Students will be provided with a packed lunch and any resources they will need for the school day. We would ask you to ensure no packed lunches or equipment is brought from home. Students should wear their normal school uniforms.

If you have a year 6 student that wishes to attend that is currently still working from home, please email office@perton-middle.staffs.sch.uk and let us know in advance before they come to school so we can ensure appropriate staff ratios and maintain the best provision possible.
