We would like to make available an option to purchase a Year 2 Leavers T-Shirt.  As in previous years, we would like to have all of the children’s first names (and initial of their surname where appropriate) on the back of the t-shirt. For this reason, we are asking parents/carers to provide consent for this. If we do not get consent from you, we cannot include your child's name on the t-shirt.

The T-shirt will be white and will have Year 2 children names that we gain consent to use on the back, as well as some artwork of the three class birds drawn by children from the classes. We will be using a local professional company to print the T-shirts.

Due to GDPR we must ask for your consent to include your child’s name on the t-shirt, regardless of whether you plan to purchase one. Please fill in this form to give your consent and to tell us how your child would like their name to appear, e.g. first name, first name with surname initial, or how they are known in school. Please no full surnames.

Even if you may not purchase a t-shirt, we would still like to try and include all the children's names if possible.
