Please complete this form if you plan to attend the senior candlelight ceremony on Thursday, June 3rd, from 9:40-10:00 a.m. The ceremony will last for about 15 minutes. Please arrive by 9:30 to find a seat and allow time to have your temperature taken. All parents attending will need to have their temperature taken before entering the sanctuary. We ask that only parents of seniors attend (no additional family or friends). We also ask that you remain socially distanced and wear your mask at all times. Thank you!
