Dear Parents and Guardians,

Governor Baker's office and the Department of Elementary and Secondary Education (DESE) released their initial guidance about reopening schools this Fall.  An email was sent with the guidance to all families on June 25th.  Please read the guidance from the state PRIOR to completing this survey.  You can access the DESE guidance at http://www.doe.mass.edu/covid19/.

We are asking that each family complete one survey if the answers for all students in your family are the same. If you would like to choose different choices for your children in various grade levels, please feel free to submit more than one form.  It is vitally important to start collecting actual numbers to plan accordingly.  Winchendon Public Schools would like to thank you for taking the time to complete the survey(s).
