Use this form to request a 2 hour block of time in the Reference Room. Limited to LPTS Students and Faculty only. A maximum of 6 patrons will be allowed for each 2-hr block. Requests must be made 24 hours in advance. Availability: Mo, Tu, We, Th from 12:30 p.m. - 2:30 p.m and 3:00 p.m. - 5:00 p.m. On We, Th. 5:30 p.m. - 7:30 p.m.
 
By making this reservation, you agree with the following:
• Agree that reservation is being made for self - LPTS Student, Staff, or Faculty only
• Agree to show up ONLY if appointment is confirmed - we will let you know by email
• Agree that only person named on reservation will show up - please, no child or spouse, friend, etc.
• Agree to provide a picture identification - a valid LPTS ID or a valid Driver License, Passport - and allow it to be retained by Desk staff and to be returned after space has been cleaned
• Agree to take temperature, answer all sign in questions and sign designated Facilities sign-in log in lobby
• Agree to wear a mask during entire time in the Reference Room
• Agree to stay in Lobby, Reference Room - and use only the Restroom on the main floor
• Agree to place books handled in reshelving bin for 7 day quarantine period
• Agree to thoroughly clean/sanitize space before departing
• Agree to remain in designated seating - i.e. not move chairs while studying and agree to maintain 6 feet of social distancing from other people at all times.

Those not following rules will be noted and asked to leave.

o COVID-19 Infection rate reaches 25+cases per 100,000 people in Jefferson Co.*  IMPORTANT: Please note that current rates are higher but we are waiving this requirement currently to offer this service
o If mandated by state, city, or LPTS administration
o Any individual who tests positive for COVID 19 has been known to enter the library building while infected
        with the virus
