Drama Kids Free Trial Class Sign-Up 2019 - 2020
To request your FREE TRIAL class simply complete this form. Your free trial is only guaranteed upon receipt of a confirmation e-mail from our office. To be allowed to attend you must show a  copy of your confirmation e-mail as your "ticket" to class.  Instructions will be included in your confirmation e-mail.  
*Please be aware we are unable to process same-day free trial requests.  If you are interested in trying a class that meets on the day you are filling out the form, please submit a request for the following week.  Thank you!

Free trials are for first-time guests only. A student cannot attend more than one free class. No cash value. There will be no credit if your child misses the free class.  Once a class fills, it will no longer be available for registration

In and for consideration of my child’s participation in this program, I hereby agree and promise that we will not hold the School District, or Drama Kids International and its employees, or any instructors responsible for any loss, damages, or personal injuries that my child may receive as a result of participation. This waiver of liability expressly includes transportation to and from, or in connection with, said program.

RELEASE STATEMENT  In consideration of accepting the above named participant(s) in our program, the undersigned parent or guardian hereby agrees to indemnify and keep harmless Drama Kids International, Inc. and its franchise owners, and employees, collectively referred to as “DKI” against any and all liability claims, judgments, or damage arising as a result of participation in our program, or as a result of any course of instruction or supervision given the participant by DKI. Furthermore, DKI is not responsible for the supervision of participant(s) either before or after the participant’s regularly scheduled class time.
By filling out this form it confirms the above and your email response will serve as an electronic agreement to said policies.
