If you are interested in membership in MCHS Beta Club, please complete the following form. If you were in MCHS Beta last year, you will get a separate form. PLEASE NOTE: IF YOU WERE IN BETA CLUB IN MIDDLE SCHOOL, YOU MUST COMPLETE THIS FORM. JR Beta (middle school) and SR Beta (high school) are two completely separate entities, and membership does not automatically carry over.

IN ORDER TO COMPLETE YOUR APPLICATION we must verify your grades from last year. Read below to see what you need to do in order for us to verify grades. *

9th GRADE: I understand I MUST SUBMIT MY REPORT CARD in order for my application to be complete. My grades my 8th grade will be used to verify my eligibility for membership. You can copy the report card sent home to you, or you can stop by the MCHS Counseling Office to get a copy.

10th-12th Grade: I understand that my grades will be pulled in order to verify membership requirements. You do not need to submit anything.

I understand the following MCHS Beta Club requirements MUST be met EACH SEMESTER in order to maintain my membership status. *

Maintain 89.5+ average

Complete the required community service hours (9th/10th = 15 and 11/12 = 15) each semester

Complete a minimum of 3 Beta Sanctioned Events a semester (more info on this will be presented at our first meeting)

Attend monthly meetings
