Captions Complaint Form
This form can be used by any person to report violations of the Closed Captioning ordinance passed in February 2023. If you have encountered a Salt Lake City business with a public-facing TV that does not have closed captions (CC) turned on, you may use this form to bring the business to the attention of the City's ADA Coordinator for further action.
