PT Primary Care Survey
Redefine Health Education is an active supporter of team-based primary care. It is our goal to facilitate PTs functioning as first-contact neuromusculoskeletal expert in the primary care team. To do that, we need to gather information on how this is being done globally. Please complete this survey if you are a PT or supervisor who has implemented this model in their health system.

Please note that you are providing this information of your own free will. This survey is not part of a research trial and has been been approved by an IRB. This is simply a means to gather information to support and assist others who are trying to implement this model.
