Dad Dance Participation Form
CALLING ALL DADS!  Each year we have a special dad dance at our concert. Sons/Daughters and dads of all ages are invited to join us for this special occasion! This is one of the most looked forward to numbers of the show by moms!  Fill out this form to participate. Be sure to include a shirt size for dad.  (Moms, feel free to volunteer your husband’s!)

JUNE CONCERT PERFORMANCE INFORMATION:
June 19 2021

You will perform in the cast your dancers class is performing in. This performance will be on the stage! Performance times for classes will be announced in March.

Tickets can be pre-purchased for June Concert beginning at 8:00am on May1st. An email link will be sent soon.
Dad Rehearsal Schedule:
Dads are not required to attend all practices but should attend at least half dates.  A video link will be shared for at-home practice.  Rehearsal times for dad groups will be announced in March.

April 17, May 1, May 22, and June 12: times TBA


By submitting the online consent form you agree to the following:
The participation fee for the Dad dance is $30 and includes the performance shirt and rehearsals.  Your dad shirt will allow you to access backstage.  Dad dancers that want a seat in the auditorium will need to pre-purchase their ticket online.

I have read and understand the terms of the June Concert Information and I agree to abide fully by its terms. I understand that performance fees and costume fees are non-refundable.

I understand the performance fees and costume fee amounts and authorize Studio 56 Dance Center to initiate electronic payments for the balance due in full at the time of registration. A $25 late fee is added to all accounts with insufficient funds.
