Duke Pizza Lunch Order Form '23/24
Please use 1 form per student. Ordering pizza lunch is a 2 step process: 1) order via this form and 2) make payment for your order(s) - etransfer to: dukepizzalunch@yahoo.com.  (If you require another payment method please email us to discuss.).

You can make 1 transfer for all students together (please put their names in the note section).

If you are able, please order for the whole year now - this is a great help to the volunteers working on pizza lunch.  

Support is available if required to participate - please speak to a school staff member or email: dukepizzalunch@yahoo.com.

Make sure you select the correct teacher for each student - parent volunteers who run pizza lunch do not have access to class lists.
