I can be flexible with when my child is at Extended Care. If there is someone else who is wait listed and needs Extended Care for their child, I will find other arrangements so their child can have a spot in Extended Care. *
I understand that by filling out this form I must prepay for the Extended Care hours I am requesting to hold my child's place on the list. Extended Care is $4/hour ($2/30 minutes the child is present). Please contact Mrs. Mullen if you have questions about your total. (Payments for the first week of school can be brought at Meet Your Teacher). *
I understand that I need to give at least 24 hours notice if I do not need care for a date I signed up for, or need care on a date I did not sign up for. I understand a fee may be charged. *
I understand that Extended Care will be filled in the order CLS receives these forms and that my child may be wait listed if CLS cannot accommodate the number of children signed up for that day/time. *
