2024 High School All-District Band Clinic Registration Form - Registration Deadline Friday, January 19th at 5:00pm

Please be sure to read the Registration Instructions and Information for the CDBA All-District Clinic Band posted on the NCCDBA web page before completing this form. Please proof your students' names as the concert program will list the participants' names exactly as they are entered here.

Directors are required to bring the registration fee ($35/per student) to the clinic. We will accept cash or booster or school checks. Make checks payable to CDBA. WE WILL NOT ACCEPT PERSONAL CHECKS!

A copy of this form will be emailed to the director's school email address as entered below as verification that the online portion of the registration process is complete.

If you are registering more than 15 students, you will need to fill out the registration form multiple times.
